# Shoulder Pain When Lifting Overhead: What's Actually Going On?

Have you ever reached overhead to grab something from a shelf, lifted weights at the gym, or put something away only to feel a sharp pinch or uncomfortable ache in your shoulder?

If so, you're not alone!

Shoulder pain during overhead movements is a common complaint, especially among people who enjoy staying active, working out, or performing repetitive movements throughout their day.

But here's something important to understand: **Just because your shoulder hurts doesn't necessarily mean something is torn, damaged, or that you need to stop doing the activities you love.**

At **Bee Well Physical Therapy in Medina**, we help individuals better understand what's contributing to their discomfort and develop personalized strategies to get them moving comfortably and confidently again.

Let's break down what might actually be happening when your shoulder hurts during overhead movements.

## Why Does My Shoulder Hurt When I Reach Overhead?

Your shoulder is one of the most mobile joints in your body. That mobility allows you to reach, lift, throw, push, and pull in many different directions.

However, all that movement requires coordination between your shoulder joint, shoulder blade, surrounding muscles, and upper back.

When something in that system isn't working as efficiently as it could, certain movements may become uncomfortable.

Some common factors that can contribute to shoulder pain include:

- **Limited shoulder mobility:** Restrictions in your shoulder's range of motion can make reaching overhead more challenging.
- **Muscle weakness or reduced endurance:** The muscles supporting your shoulder may not have the strength or endurance needed for certain activities.
- **Shoulder blade movement:** Your shoulder blade plays an important role in overhead motion, and changes in how it moves may contribute to discomfort.
- **Training or activity changes:** Suddenly increasing weight, repetitions, or activity levels can place more demand on your shoulder than it's currently prepared to handle.
- **Rotator cuff irritation:** The muscles and tendons of the rotator cuff help stabilize and move your shoulder. When these tissues become irritated, overhead movements may become painful.

It's important to remember that **shoulder pain isn't always caused by one specific issue.** Often, several factors contribute to how your shoulder feels and functions.

## Is It My Rotator Cuff?

When people experience shoulder pain, one of the first things they think about is their rotator cuff.

And while the rotator cuff can certainly be involved, it's not always the source of the problem.

Your rotator cuff consists of four muscles that work together to help stabilize and move your shoulder. These muscles are especially important during activities that involve lifting, reaching, and controlling your arm.

Sometimes, shoulder pain may be associated with rotator cuff-related conditions, including tendon irritation or tears.

However, **experiencing pain doesn't automatically mean you've torn your rotator cuff.**

In fact, changes in tendons and other shoulder structures can sometimes appear on imaging even in individuals who aren't experiencing any pain.

This is one reason why understanding your symptoms, movement, strength, and activity history is so important.

Rather than focusing solely on what an image might show, a physical therapist can help evaluate how your shoulder is functioning as a whole.

## Should You Stop Lifting If Your Shoulder Hurts?

Not necessarily!

One of the biggest misconceptions surrounding shoulder pain is that you need to completely stop exercising or avoid using your shoulder until the discomfort disappears.

While some activities may need to be temporarily modified, avoiding movement altogether isn't always the best solution.

In many cases, **finding ways to continue moving within a tolerable range can be an important part of recovery.**

Depending on your symptoms, this might mean:

- Reducing the weight you're lifting.
- Adjusting your range of motion.
- Modifying certain exercises.
- Temporarily decreasing training volume.
- Building strength and gradually returning to overhead movements.

For example, if overhead presses are uncomfortable, you may be able to continue strengthening your shoulders using a different exercise variation while working toward your original movement.

The goal isn't necessarily to eliminate every challenging movement.

**It's to help your shoulder become better prepared to handle those movements again.**

Sudden severe pain, significant weakness, or pain following an injury should be assessed before continuing strenuous activity.

## What Does Physical Therapy Do for Shoulder Pain?

When it comes to shoulder pain, there's no one-size-fits-all solution.

Two people can experience discomfort during the exact same movement but have completely different factors contributing to their symptoms.

That's why an individualized approach matters.

At **Bee Well Physical Therapy in Medina, Ohio**, we focus on understanding the bigger picture rather than simply treating where it hurts.

Depending on your symptoms and goals, a physical therapy evaluation may include:

**1. Understanding Your Symptoms**

We'll discuss when your pain started, which movements aggravate it, your activity levels, and what you're hoping to get back to doing.

**2. Assessing Your Movement**

Looking at shoulder mobility, strength, shoulder blade movement, and upper back mobility can help identify areas that may benefit from additional attention.

**3. Identifying Contributing Factors**

Whether you're lifting at the gym, working overhead, or reaching throughout your daily routine, understanding the demands placed on your shoulder helps guide your treatment.

**4. Building an Individualized Plan**

Your treatment may include targeted strengthening exercises, mobility work, activity modifications, and a gradual return to movements that have been uncomfortable.

**5. Helping You Stay Active Long-Term**

Our goal isn't just to help you feel better temporarily. We want to help you build the strength, confidence, and understanding needed to continue doing the activities you enjoy.

## When Should You Seek Help for Shoulder Pain?

Occasional soreness after an unfamiliar workout isn't necessarily a cause for concern.

However, if your shoulder pain continues to interfere with your daily activities or workouts, it may be time to have it evaluated.

Consider seeking help if:

- You consistently experience pain when reaching or lifting overhead.
- Your shoulder discomfort is limiting your workouts.
- You've noticed changes in shoulder strength or mobility.
- Your symptoms aren't improving despite modifying your activities.
- Shoulder pain is interfering with your sleep or everyday tasks.

Seek prompt medical attention for significant pain after a fall or injury, a visibly deformed shoulder, or sudden inability to lift your arm. Shoulder pain accompanied by chest pain, shortness of breath, or sweating requires emergency evaluation.

**You don't have to wait until your pain becomes unbearable to seek guidance.**

Addressing symptoms early can help you better understand what's happening and determine the next steps.

## Shoulder Pain Doesn't Have to Keep You From Doing What You Love

Whether you're an athlete, a regular gym-goer, or someone who simply wants to reach into the kitchen cabinet without discomfort, shoulder pain can be frustrating.

But experiencing pain with overhead movements doesn't automatically mean you need to give up lifting, exercising, or staying active.

With the right guidance, appropriate modifications, and an individualized plan, many people can improve their shoulder function and work toward returning to the activities they enjoy.

At **Bee Well Physical Therapy in Medina**, we believe in helping people understand their bodies, build confidence in their movement, and take an active role in their recovery.
